Intact Canal Wall Tympanoplasty with Mastoidectomy (Second report)

Abstract
These data were based upon clinical studies of 115 cases, nha had undergone intact canal wall iympanoplasty with mastoidectomy during the one year period dating from July 1, 1968 through June 30, 1969 at Presbyterian Medical Center E. N T. dept.
The results were as follows:
1. The incidence of tympanoplasty was highest in the young age group from 10 to 30 years old.
2. 1.7% of all patients failed the clinical function test of eustachian cube.
3. Under the surgical microscope the majority of cases were found to have moderate hypertrophy of middle ear mucosa, but in 13961 the middle ear mucosa was either normal or mildly hypertrophied.
4. The frequency of cholesteatoma was 28.5.6 and they were associated with wide destruction of ossicles, except in 4 cases.
The range of hearing loss in terms of pathology was as follows:
a. Drum perforation: most common type-total. 49.2¡¾18.25
b. Cholesteatoma: most common location-epitympanum. 47.6¡¾18.4
c. Osseous destruction: most commonly involved part; malleous and incus. 57.4¡¾9.5
5. The frequency of tympanosclerosis was 24.4% of all cases. 53.6% of these cases had ossicular fixation and there was a high incidence in female (60.4%). Mucoid or no discharge was noted.
a. Drum perforation: most common type-central. 55.3¡¾15.2
b. Tympanosclerosis: most common location-drum. 42.5¡¾18.5
c. Osssicles: most commonly normal. 44.5¡¾16.05
6. Pure otitis media without cholesteatoma and tympanosclerosis was present in 47%, of all cases and hearing loss was shown to increase with the size of perforation and ossicular destruction.
The range of hearing loss in relation to pathology was as follows:
a. Drum perforation: most common type-central. 37.02¡¾19.12
b. Ossicles: most commonly normal. 30.5¡¾8.2
7. The frequency of ossicular fixation was 19% of all cases, and tympanosclerosis w,s tl:e most common cause. The hearing loss Nvas almost same whatever the cause.
